- Requirements: 16 & Older: Form DS-11, completed in black ink only (forms available at the library, or complete online and print here.) Certified copy of Birth Certificate Current ID (Driver's License, etc.) Passport photos (must be current, color, full-front view of the face, and 2"x2" in size). 0-15 Years Old: Form DS-11, completed in black ink only (forms available at the library, or complete online and print here.) Certified copy of Birth Certificate Both parents and child must be present at appointment If one parent is unable to be present, that parent must fill out a DS-3053 form (available here) and have it notarized. This completed & notarized form must be submitted at the time of the passport appointment Passport photos (must be current, color, full-front view of the face, and 2"x2" in size)
